CHOICE FREEDOM MEMBER AGREEMENT
This page (together with the documents referred to on it) contains the terms and conditions of your membership known hereafter as Choice Freedom and Choice Freedom Benefits Plan.
Choice Freedom™ website is located at CHOICEFREEDOMCARD.CO.UK (the "Website"). This is a site operated by Choice Member Benefits Limited ("we" or "us"). We are registered in England and Wales under company number 7705088 and we have our registered office at Riverside House, 44 Wedgewood Street, Aylesbury, Buckinghamshire, HP19 7HL. Our main trading address is 1707 Warren Road, Indiana PA, 15701, United States of America. Our VAT number is 120 5130 79.
The Choice Freedom™ is an online merchandise shopping service. After obtaining an Account, you can make purchases of select items available at or through the Service. Accounts may be obtained by individuals 18 years of age or older, who possesses a valid and active credit card, debit card, bank or building society account. We will consider applications regardless of your credit history but please consider your overall financial position before applying.
THE CHOICE FREEDOM CREDIT CARD IS NOT A VISA, MASTERCARD, OR DEBIT CARD. CHOICE FREEDOM ACCOUNT IS A LINE OF CREDIT THAT CAN BE USED BY AN ACCOUNT HOLDER TO SHOP EXCLUSIVELY AT OUR ONLINE SHOPPING WEBSITE. NO DEPOSIT HAS TO BE PAID FOR PURCHASES AT OUR WEBSITE. CHOICE FREEDOM™ DOES NOT FACILITATE, NOR PROVIDE ASSISTANCE IN OBTAINING CREDIT FROM ANY OTHER CREDIT-ISSUING ENTITIES. CHOICE FREEDOM™ DOES NOT CHARGE AN ADVANCED FEE TO ESTABLISH CREDIT, NOR DOES CHOICE FREEDOM™ ATTEMPT, OR CLAIM TO ATTEMPT TO REPAIR A CUSTOMER'S CREDIT HISTORY.
How it Works: To obtain a credit card account (an "Account"), you must first submit an application to Choice Freedom™. When you apply for an Account, you agree to provide accurate, current and complete information about yourself ("Application Data") as prompted by our application form. You will also be required to enter into a credit agreement with us in relation to your Account. A copy of the credit agreement together with associated documents will be provided to you when you have completed the application form. Please make sure that you read the documents provided carefully before submitting your application. You understand that by submitting an application to Choice Freedom™, you are not guaranteed to obtain an Account. Choice Freedom™ reserves the right, in its sole discretion, to accept or reject any application for any reason, or for no reason, whatsoever.
Incomplete Applications: By clicking "submit" on the application page located at HTTPS://CHOICEFREEDOMCARD.CO.UK, you grant permission to Choice Freedom™ to contact you, when necessary, via e-mail or telephone to complete your application.
If Choice Freedom™ approves your application; you will receive an e-mail confirming your application approval and your associated username and password information ("Login") information. Once you receive this Login information, you may make immediate use of the Service.
You agree to maintain and update your Application Data to keep it as accurate, current and complete as possible. Choice Freedom™ reserves the right, in its sole discretion, to terminate your Account for inaccurate, untrue, deceptive and/or incomplete Application Data. In order to update your Application Data, please access and update your online profile. Upon Account activation of your membership, you will receive various benefits as well as a credit card account to use exclusively on purchases available through the Choice Freedom Outlet Website. Your Account will not work at any other website. You do not need to pay a deposit but there are nominal delivery and handling fees required for each purchase. These are payable by debit or credit card at the time you submit your order. You will receive a physical credit card within (5) five days to (2) weeks of your account activation. If you do not receive your card, please contact our customer services department.
Purchases, Payments and Monthly Billing: If any debit request for a fee associated with your Account is returned due to non-sufficient funds available in your bank account, Choice Freedom™ may attempt to collect the applicable fee by automatically resubmitting to your bank subsequent debits. Any subsequent debit attempts by Choice Freedom™ will not exceed the original fee amount. Furthermore, you understand and agree that if Choice Freedom™ is unable to obtain payment from your bank, Choice Freedom™ reserves the right, subject to sending you any statutory notices required by law, to terminate your Account or reduce your credit limit until a consistent payment history has been established.
Credit Bureau Reporting: Choice Freedom™ will report your new Choice Freedom/Choice Freedom Outlet Account to at least one major credit reference agency upon receiving positive confirmation of your identity. In order to confirm your identity, you may be required to send Choice Freedom™ additional information. The Choice Freedom Account does not guarantee aid in building or re-building credit, or to the posting of any positive references to any credit report. Due to circumstance beyond our control, it may take up to 60 days for your Choice Freedom information to appear on your credit report. Further details about how we and the credit agencies may use your personal information are set out in our privacy notice.
Monthly Membership/Benefits Plan: Upon activation of your Account, all Choice Freedom members are automatically enrolled in our monthly membership benefit plan. Upon activation with Choice Freedom™, you are bound by the User Agreement provided at the enrollment site. Please read this carefully www.choicefreedomcard.co.uk. Monthly membership includes access to Dental Discounts, Optical Discounts, Aesthetics Discounts, Hearing Discounts, Roadside Assistance and Legal Service. For ease and convenience, the £14.95 benefits plan membership fee will be auto-debited monthly from the debit or credit card account of which you supplied details to us. In the event your monthly auto-payment of £14.95 fails three times due to insufficient funds, we will for your convenience auto enroll you into our basic starter benefits plan that in addition to your regular account and credit line access includes Dental Discounts, Optical Discounts, Aesthetics Discounts, Hearing Discounts and Legal Service at a reduced charge of £4.95 per month. Full benefits to include the Roadside Assistance component can be accessed by contacting customer service and requesting that we reinstate full benefits at £14.95 per month and by making proper payment arrangements to include any missed back payments.
Plan Limitations and Restrictions: Access to member benefits is restricted to only the member on file of an active Account. Should you wish to add additional family members to your plan, please contact customer service for pricing. Fraudulent use and access of benefits constitutes theft and will be prosecuted to the fullest extent of the law. Choice Freedom™ has the right to temporarily suspend or cancel member plans that have the appearance of fraudulent use. It is your responsibility to know your plan limitations. Upon reaching all or any single plan inclusion program limit within a 12-month period, access will be denied.
Good Standing Requirement: You understand that you must maintain your Account in good standing by making at least the minimum monthly payments on time and accurate account and personal information. If your Account is in default status, you understand that your Account privileges and benefits may be suspended and/or terminated until you have re-established a consistent pattern of on-time monthly minimum payments.
Cancellation and Refund Policy: Choice Freedom™ strives to provide exceptional customer service. Cancellation and refund requests in relation to the Monthly Benefits Plan can be made within 30 days of your statement date by calling Member Services at 0808 234 0335, Monday through Friday from 9:00 a.m. to 12:00 a.m. GMT, and Saturday from 3:00 p.m. to 8 p.m. GMT (excluding holidays). Refunds can only be obtained by calling this number. Cancellations made beyond 30 days from your statement date are not eligible for a refund.
Fraud Resolution: As a matter of policy, we cooperate fully with all law enforcement agencies in the pursuit of fraud, with regard to the unauthorised use of information in order to apply for this membership. If you believe your Account was fraudulently charged for the application and processing fee, we will make every effort to aid in a resolution. Before we can take any action, please fax or mail a copy of the official report filed with your local Police detailing the suspected fraudulent act and also a copy of the portion of your account statement showing the date and amount of the withdrawal. Electronic signatures on documents hold equivalent legal status as traditional handwritten signatures. By completing the online application at the website for Choice Freedom™, you certify that your digital signature is the equivalent of your handwritten signature.
Customer Complaints: If you feel at any time you are not receiving the customer care and attention you deserve, we encourage you to contact our 'Customer Advocacy' department.
